The following carriers have released updated enrollment forms for new small group business. Please be sure you are using the updated forms for the effective dates indicated below.
Aetna has updated their Employer Application and Employee Enrollment/Change Form for 2018. The pending regulatory approval watermark has been removed and plan names are updated with the CA SB137 compliant medical product names. The new forms should be used beginning with 3-1-2018effective dates.
Blue Shield has released their updated Employer Master Group Application and Employee Enrollment Form to be used for 4-1-2018 and later effective dates – prior forms will not be accepted. The new employer application reflects the ability to offer the Full and Tandem PPO networks alongside each other beginning with April business.
CaliforniaChoice has released their new Q2 Employer Application, Q2 Employee Enrollment Application, Q2 Employee Change Request Form and an updated ACH Payment Form. All must be used starting with 4-1-2018 effective dates – prior forms will not be accepted.
